Question?
Has anyone seen a perforations in a stainless steel boiler tubes?
Caused by contaminants entering the combustion chamber causing perforations?

Yes.
When the combustion air is contaminated with chemicals, such as swimming pool heaters getting air from the same room the pool chemicals are stored.
But usually the burner tubes go first.
If they have been changed, you may want to look for some sort of corrosive atmosphere.
Not all stainless steel, is."Iron sharpeneth iron..."
