    https://hvacinstitute.ferris.edu/src/608.php

I did the same thing passed my EPA certification in Nov, 1994 & received my card from Ferris State University. Have been searching the internet the last few days & found this website, but did not seem to have any answers. Last night I stumbled upon another website which I thought may have replacement cards, but they were no longer in service. However they did provide a toll free phone# 866-880-7674 to a dept at Ferris State University who was able to provide me with a replacement certification card. I just called them today & they will be mailing it to me for a fee (ranges from $20 - $30) depending on what type of card and/or certificate you want. Have not received it yet, but it sounds like the real deal. Give them a call if you need a replacement card. They did have my info on file & will be sending me a card. Good luck.

At job interview and if you want to buy stuff at Abco and Johnstone without an employer, they asked me for it to open an account. I gotta get a new card myself, my certificate uses my SS#..
