Does it have a Honeywell Valve? They were wellknown for this problem. If so your only choice is to change out the valve. Contact Vermont Castings group for the kit number.
As the title states I have looked at a fireplace, the pilot stays lit when on pilot, as soon as you turn valve to " ON " it completely shuts down.
The thermopile is generating 525MV with the wall switch off, about 300 with the switch on.
Could this be a vent switch (i'm think the vent switch would prevent the pilot form coming on but not 100% sure ?)
The unit has been idle for a couple of years. I have checked the main orifice and burner tube and they are free of debris.
I'm leaning towards faulty valve but looking for opinions as I am typically a furnace guy, trying to help a buddy out.

Thank you for the response it is very much appreciated. Not sure which valve it has, didn't reaize this model came equipped with different valves until doing some research. I will be checking which one it is.
If the vent switch is tripped the pilot will not light, correct?
Pilot will light but drops out as soon as you let up on the know. Honeywell valves had the sparker button (square) in the center of the valve between the pilot knob and the hi low
Your main valve coil could have a short in it thus robbing all of the voltage. I had one very similar and this was the case started out as intermittent and then gradually happened more often until it would not stay lit at all when turned to on position it could also be any of the switches that are in series they too could be robbing the voltage
I had the exact same problem last week. Changed the thermopile. Now I got about 720mv with switch off. Didn't check with switch on because it was working fine with the new thermopile.
