    Here is an update from Karen and Bret........a little extra help from the big guy upstairs after our knelt requests guys.......

    It was a fairly quiet summer for us through the end of August.

    In late August, Bret’s LDH (tumor marker) was rising and wanted to know why. Bret had an MRI of the brain and PET CT in early Sept. Confusing radiology read but the good news was that the radiation oncologist did not feel or agree with the radiologist that there were new brain tumors from the MRI taken two months before, so no changes there. The PET CT showed that for torso tumors, appear to be no longer visible in his lungs, intestine tumor about the same, and liver tumors show some larger, some no longer visible, some decreased in size, and one or two that increased in size that oncologist felt were becoming more aggressive.
    Oncologist upped Bret’s meds in Sept by 50%, he has been super tired. A few skin issues but balanced out with steroids so far.

    Unfortunately, on Sept 29 found out his LDH doubled since the end of August even with increased meds. This is discouraging news, especially since he spent all of September turning up the dose to the maximum. Have been looking at next steps for us.

    After talking to oncologist, she feels that the sharp increase in the LDH is due to the large tumor in the liver. The decision has been made to do a procedure called Radioembolization (Y90), where inject Yttrium into the tumor vessels to kill its blood supply. Is a type of vascular intervention. Is a two part process, one day outpatient surgery to map the tumor and vessels, and two weeks later another one day surgery to do the injection/embolization. It has been used for awhile for hepatitis C and a few other cancers however not being used for metastatic melanoma of the liver. On Oct 12 we go in for mapping then in 2 weeks hopefully will do the procedure. It’s not FDA approved for melanoma, waiting to hear whether insurance covers. But it’s a go for us either way.

    Oncologist also discussed two clinical trials, I am already on the list for these. And another option could be some variations on treatments we have used in the past. The FDA has also recently approved one new treatment. As of this week, am experimenting on some new dosage ideas for taking the current treatment I am presently on.

    It's been a wild ride this last month-and-a-half. However, I was thinking this morning a lot of what we're doing we are the first or the first few to try - no matter what, it's great to know that we're helping future cancer patients.

    Thanks to everyone who has helped in so many ways including driving to the cancer center and other places, and helping take care of small projects we have trouble getting done. Having such great friends next to you for this whole experience, you learn how amazing people are!

    Below, the vascular intervention description, going to start in 2 weeks. Will be super tired for 4 weeks after the second step, but not sure how much different that will be from my “normal” tired.

    Radioembolization is a minimally invasive procedure that combines embolization and radiation therapy to treat liver cancer. Tiny glass or resin beads filled with the radioactive isotope Yttrium Y-90 are placed inside the blood vessels that feed a tumor. This blocks the supply of blood to the cancer cells and delivers a high dose of radiation to the tumor while sparing normal tissue.
