Problems with munchkin

I have a six apartment building that we installed a Munchkin #199masme ri boiler with a ssu-60 super store.We were told that this thing would never run out of hot water for the domestic side but we have had trouble with it since they installed it a little over a year ago.It has failed a few times and shut down all together--The guys who put it in can not figure out what the problem is ,they tried re piping the system but it was worse so they put it back they way it was.A friend of mine who knows this stuff says the super store is to small for that size building.Anyone out there have any ideas?Thank you and look forward to any help we can get,Tom.

Is the indirect a priority zone. Are you using OD reset.
What temp is the indirect set to, does it have a tempering valve.
Are you using zone valves or circs. What size piping to the indirect.
