Help Identify Old Lennox

I have a Lennox HP16-461Y-11P heat pump. I do not have any details/specifications and will appreciate if someone can provide me the information. The heat pump is running good for last 24+ years but I feel someday I would need to replace it. Lately a hivac switch (as told by a service person) get tipps off often. Could it be the thermostat casuing this problem?
