It seems like there is a request for functional testing forms every so often, and I just stumbled on this site.

I have been using this site for about 4 months now and I really like it. Although I had to alter the forms to my specific needs and applications for my area. Last time I had to make functional testing forms for a LEED project it took me almost 2 weeks. Now it only takes me 2 days.

I put the link up in a couple of other threads for guys asking about commissioning documents. PECI has some great forms for equipment and systems commissioning, but I feel that they don't have a comprehensive DDC/BAS set.

I like to be extremely detailed in my DDC documents (Point to Point / Comfiguration and Sequence). I like to see PID values, input offsets, jumper positions (inputs, baud rate, outputs, address etc.) scale ranges etc. I can't remember how many times I have had "warranty calls" and when I get onsite and check the system variables against my docs and see that some in the facility changed PID values or other configurations.
