Last one we did, the supplier used http://www.kcccurbs.com/

It fit like a glove, wasn't nearly as tall as the same match he did last year with a different vendor.

Those look a lot like the ones we make in our shop.
90% of the time if it's a custom curb our shop will just make it. ;)
