Lost EPA card

I am looking to move and have discovered that I lost my EPA certification card. Does anyone know who I can contact to receive a copy of my certification?

You will need to know the name of the testing place that give the test to you. The place that mine was from was the esco institute and you can go online to their web site and print one out or request a new one. I yours is differant maybe harder good luck. I don't know any other way

It mightbe easier to rewrite it, I think my 10 year old could probably pass the test.

I lost mine, called ESSCO or whatever and they sent me a new one after I sent them $10 and a copy of my SSN.
